The hidden causes of hypertension: from inflammation and gum health to pollution and gut bacteria

High blood pressure, also known as hypertension, has long been linked to familiar lifestyle factors such as eating too much salt, lack of exercise, smoking, alcohol consumption, stress and being overweight. While these factors remain important, scientists are now discovering that the causes of high blood pressure are far more complex than previously believed.

New research suggests that the immune system, oral health, air pollution and the gut microbiome may all play a major role in controlling blood pressure. These findings are changing how experts understand hypertension and could lead to new ways of preventing and managing the condition.

Around one in three adults worldwide lives with high blood pressure, affecting an estimated 1.5 billion people. It is one of the leading causes of premature death globally because it increases the risk of heart disease, stroke, kidney failure and other serious health problems.

The biggest challenge is that hypertension often has no obvious symptoms. This is why it is frequently called a “silent killer”. Many people do not know they have it until complications develop.

Why high blood pressure is more complicated than “bad plumbing”

For decades, scientists viewed hypertension mainly as a problem involving the heart, kidneys and blood vessels. According to this traditional explanation, blood pressure rises when the body experiences a combination of genetic risk, unhealthy lifestyle choices and changes in the cardiovascular system.

However, researchers now believe this explanation does not tell the full story.

Professor Tomasz Guzik, chair of cardiovascular medicine at the University of Edinburgh, has been studying the role of the immune system in hypertension. His research suggests that immune cells may be a missing link between lifestyle factors, inflammation and rising blood pressure.

In early experiments, researchers found that mice without a functioning immune system were protected against severe hypertension. When immune cells called T lymphocytes were restored, the animals developed high blood pressure.

Further research in humans has supported the idea that the immune system can contribute directly to hypertension.

When immune cells become overactive, they can enter blood vessel walls and release inflammatory chemicals. This inflammation can cause blood vessels to become stiff, making it harder for blood to flow. As a result, pressure inside the vessels increases.

Over time, this can damage organs. The heart must work harder, increasing the risk of heart failure, while the kidneys may retain more salt and fluid, further raising blood pressure.

Gum disease may increase blood pressure risk

One surprising area of research is the connection between oral health and cardiovascular health.

Bleeding gums, redness and swelling are signs of inflammation. Although mild gum disease is common, untreated gum problems can progress into periodontitis, a serious infection of the tissues supporting the teeth.

Scientists have found that bacteria linked to gum disease may trigger inflammation throughout the body. Using data from the UK Biobank, researchers found evidence suggesting that gum disease may not simply occur alongside high blood pressure but could actually contribute to it.

A clinical trial explored whether treating gum disease could improve blood pressure levels. People with both hypertension and periodontitis received either intensive gum treatment or standard cleaning.

Those who received deeper treatment had healthier gums, reduced inflammation and lower blood pressure compared with the control group.

The findings suggest that looking after your teeth and gums may be another important strategy for protecting heart health.

Simple habits such as brushing twice daily, cleaning between teeth and attending regular dental check-ups may have benefits beyond preventing cavities.

Traffic pollution and blood pressure

The air we breathe may also influence our cardiovascular health.

Traffic pollution contains tiny particles that can enter the lungs and trigger inflammation throughout the body. People living near busy roads may have greater exposure to these pollutants.

Professor Douglas Brugge from the University of Connecticut studied residents living near major roads in Somerville, Massachusetts. Participants used high-efficiency particulate air purifiers in their homes, and researchers monitored changes in blood pressure.

The study found that people with existing hypertension experienced an average reduction of around 3 mmHg when using genuine air purifiers compared with devices without effective filters.

Although the reduction may appear small, even modest improvements in blood pressure can reduce the long-term risk of heart attacks and strokes.

For people living in areas with heavy traffic pollution, improving indoor air quality may become one additional tool for managing cardiovascular risk.

The gut microbiome connection

Another emerging area of research focuses on the trillions of microorganisms living in our digestive system.

For years, scientists have known that diets rich in fibre are associated with better health, including lower blood pressure and reduced risk of heart disease. However, researchers are now discovering why.

Professor Francine Marques from Monash University has shown that gut bacteria may help regulate blood pressure through molecules called short-chain fatty acids.

When beneficial gut bacteria break down fibre, they produce these compounds, which can reduce inflammation and support healthier blood vessels.

Clinical research has shown that diets designed to increase these beneficial compounds may lower blood pressure. In one trial, participants consuming fibre enriched with short-chain fatty acid producing compounds experienced a reduction in blood pressure similar to the effect of some blood pressure medications.

The message is clear: feeding your gut bacteria with fibre-rich foods may help support healthy blood pressure.

  • Fruits
  • Beans and lentils
  • Nuts and seeds
  • Whole grains
  • Oats

Most adults do not consume enough fibre each day, making this a simple area where dietary changes can have meaningful benefits.

How to lower your blood pressure naturally

While researchers continue exploring these new discoveries, established strategies remain essential.

To help keep blood pressure within a healthy range:

  • Check your blood pressure regularly
  • Exercise consistently
  • Maintain a healthy weight
  • Reduce excessive salt intake
  • Avoid smoking
  • Limit alcohol consumption
  • Prioritise quality sleep
  • Manage stress levels
  • Eat a fibre-rich diet
  • Maintain good oral hygiene
  • Reduce exposure to air pollution where possible

Some risk factors, such as genetics and living near busy roads, cannot always be changed. However, new research shows there are many practical steps people can take to support healthier blood pressure.

The future of hypertension research

High blood pressure is no longer viewed simply as a problem of damaged arteries or an overworked heart. Scientists are uncovering a much more connected picture involving the immune system, inflammation, the mouth, the environment and the gut.

These discoveries could lead to new treatments and prevention strategies in the future. For now, the best approach remains combining medical advice with healthy lifestyle choices.

The first step is simple: know your numbers. Regular blood pressure checks can help identify problems early and give you the opportunity to take action before serious complications occur.
