A new scientific study has revealed a promising approach that could transform the treatment of glioblastoma, one of the most aggressive and deadly forms of brain cancer. Researchers have identified specific periods immediately after brain tumor surgery when chemotherapy may work more effectively than under current treatment practices.

However, scientists from The University of Manchester believe there may be a better approach.

Their research found that surgery temporarily disrupts the blood brain barrier, a protective layer that normally prevents many medicines from reaching brain tissue. During this brief period, chemotherapy drugs may gain better access to areas where cancer cells remain.

Researchers identified two important treatment windows:

  • Approximately 15 minutes after surgery.
  • Around 48 to 72 hours following surgery.

According to the study, delivering treatment during these periods could improve the effectiveness of cancer drugs before the blood brain barrier returns to its normal protective state.

Understanding the Blood Brain Barrier

The blood brain barrier acts as the body's natural defense system, protecting the brain from harmful substances circulating in the bloodstream.

While this protection is essential for healthy brain function, it also creates a major obstacle for doctors trying to treat brain tumors. Many chemotherapy drugs cannot easily pass through this barrier, reducing their ability to reach cancer cells.

The latest findings indicate that surgery temporarily creates an opportunity for medicines to cross into the brain more efficiently. Researchers believe this short period could become a valuable treatment window for future therapies.

How the Study Was Conducted

The research, published in Science Translational Medicine, used laboratory models designed to closely mimic the surgical removal of glioblastoma tumors.

Scientists tested medicines that would normally struggle to enter the brain unless the blood brain barrier had been disrupted.

The results showed that treatment delivered during the identified time windows significantly reduced tumor regrowth in the laboratory models.

Although these findings are encouraging, researchers emphasize that additional studies involving human patients are necessary before this strategy becomes part of routine medical care.

Lead researcher Dr. Thomas Kisby explained that the study demonstrates, for the first time, that glioblastoma surgery briefly creates a vulnerability that doctors may be able to use to their advantage.

Researchers believe that carefully timed treatment during these periods could slow or even prevent the cancer from returning.

Professor Kostas Kostarelos also noted that this strategy may allow doctors to use advanced therapies, including targeted medicines, nanoparticle treatments, immunotherapy, and gene therapy, more effectively after surgery.

Instead of relying only on developing entirely new drugs, physicians may be able to improve outcomes by delivering existing treatments at the most beneficial time.

Glioblastoma remains one of the most challenging cancers to treat because even the most successful surgeries cannot remove every cancer cell.

Cancer cells that remain near the surgical site often begin growing again within months.

If future clinical trials confirm these findings, doctors could introduce treatment much earlier than they do today, increasing the chances of destroying these remaining cancer cells before they spread.

This approach could represent a major advancement in brain cancer treatment without necessarily requiring completely new medicines.
