Thyroid Medication Recall Raises Questions About Potency and Patient Safety

A nationwide recall of a commonly prescribed thyroid medication has prompted health concerns after testing found that one batch may contain more active ingredients than intended.

Vitruvias Therapeutics Inc. announced the voluntary recall of one lot of its Thyroid Tablets, USP 30 mg on Aug. 21, 2026. The U.S. Food and Drug Administration published the company's recall notice on Aug. 24.

The recall involves Lot 504950, which contains 3,655 units of the medication. According to the FDA, 1,955 units were distributed to patients in the United States. The affected products were distributed between Jan. 31, 2025, and Sept. 30, 2025. The product carries an expiration date of Sept. 30, 2026.

The reason for the recall is the potential for the tablets to be superpotent, meaning they could contain more thyroid hormone than intended.

Why the Recall Matters

Thyroid hormones play an important role in regulating the body's metabolism and many other biological processes. When someone has hypothyroidism, the thyroid gland does not produce enough hormone, which can result in symptoms such as fatigue, sensitivity to cold, weight gain and difficulty concentrating.

Thyroid hormone replacement is commonly used to help restore hormone levels in people with an underactive thyroid.

However, the amount of thyroid hormone in a medication needs to be carefully controlled. Too little may fail to adequately treat hypothyroidism, while too much can push the body in the opposite direction and produce symptoms associated with an overactive thyroid.

The FDA says excessive exposure to the recalled product could potentially cause hyperthyroidism. Possible symptoms include unexplained weight loss, sensitivity to heat, tiredness, nervousness, muscle weakness, elevated blood pressure, chest discomfort, a rapid heartbeat and abnormal heart rhythms.

The Cleveland Clinic similarly notes that excessive thyroid hormone can accelerate several body functions. Hyperthyroidism may cause symptoms such as a fast heartbeat, sweating, anxiety, tremors, difficulty sleeping and unintended weight loss.

What Is Thyroid, USP?

Thyroid Tablets, USP are a natural thyroid hormone preparation derived from porcine, or pig, thyroid glands. The FDA says the product contains levothyroxine and liothyronine and is used to treat hypothyroidism.

The thyroid itself is a small, butterfly-shaped gland located in the lower part of the neck. Despite its size, it has an important influence on how the body uses energy.

Thyroid hormones help regulate metabolism, temperature tolerance, energy levels and other bodily functions. When hormone levels are too low, the body's processes can slow. When levels are too high, those processes can speed up.

That is why maintaining the correct medication dose is particularly important for people who depend on thyroid hormone replacement.

Who May Face Greater Risks?

According to the FDA recall notice, certain groups may be particularly vulnerable to excessive thyroid hormone exposure.

These include older adults, pregnant women and infants, especially if exposure continues over an extended period.

For older adults, excessive thyroid hormone levels can place additional strain on the cardiovascular system. Pregnancy also requires careful management of thyroid hormone because both insufficient and excessive treatment can create health concerns for the mother and developing baby.

The FDA notes that overtreatment during pregnancy has been associated with complications including premature delivery and low birth weight. In infants, excessive thyroid hormone treatment may affect growth and development.

These risks do not mean that everyone who has taken the recalled medication will experience a health problem. Vitruvias said it had not received reports of adverse events known to be connected with the recalled lot when the FDA notice was issued.

What Patients Taking the Medication Should Do

Patients who believe they may have the recalled product should first determine whether their medication matches the affected lot.

The FDA identifies the recalled product as:

  • Product: Thyroid Tablets, USP 30 mg
  • NDC: 69680-166-00
  • Lot: 504950
  • Expiration date: Sept. 30, 2026
  • Distribution: Nationwide in the United States

The most important point for patients is that they should not stop taking the medication on their own simply because their product may be part of the recall.

The FDA and Vitruvias advise patients currently taking Thyroid Tablets from Lot 504950 to contact their healthcare provider for guidance and, if appropriate, a replacement prescription.

Abruptly changing or stopping thyroid medication without medical advice can create its own problems, particularly for people who rely on replacement hormones to maintain normal thyroid function.

Anyone who develops concerning symptoms should contact a healthcare professional. Symptoms such as a racing or irregular heartbeat, chest pain, significant weakness or other sudden changes should receive prompt medical attention.

How Patients Can Check Their Medication

Patients can compare the lot information printed on their medication packaging with the details listed in the FDA recall notice.

If the package identifies Lot 504950, patients should contact their healthcare provider or pharmacist before making changes to their treatment.

Pharmacists can also help patients determine whether their prescription is affected and discuss possible replacement options.

Vitruvias said it has been contacting wholesalers and instructing them to stop distributing the affected lot. The company also said it is arranging for recalled products to be destroyed.

For questions specifically related to the recall, the FDA notice lists Vitruvias Therapeutics at safety@vitruvias.com and (256) 239-9373, Monday through Friday from 9 a.m. to 5 p.m. Central Time.

Health problems or suspected adverse reactions associated with the medication can also be reported through the FDA's MedWatch program.

Why Correct Thyroid Dosing Is Important

Thyroid conditions can be difficult to identify based on symptoms alone because many symptoms overlap with other health conditions.

The Cleveland Clinic explains that hypothyroidism may cause fatigue, weight gain, cold sensitivity and a slower heart rate, while hyperthyroidism can cause weight loss, heat sensitivity, anxiety and a faster heartbeat.

Blood testing is commonly used to assess thyroid function. Tests can include thyroid-stimulating hormone, or TSH, along with thyroid hormone measurements such as T4 and T3.

This makes medication recalls involving thyroid hormones especially important. A patient may not immediately know whether a change in symptoms is related to their underlying thyroid condition, their medication dose or another health issue.

For that reason, patients should avoid adjusting their thyroid treatment without professional advice.

The Bottom Line

The recall of Vitruvias Therapeutics' Thyroid Tablets, USP 30 mg involves a specific lot, 504950, rather than every thyroid medication or every product manufactured by the company.

The concern is potential superpotency, which means some tablets may contain more thyroid hormone than intended. Excess thyroid hormone can produce symptoms associated with hyperthyroidism and may pose greater risks to certain groups, including older adults, pregnant women and infants.

Patients should check their medication packaging and contact a healthcare professional if they discover that they have the recalled lot. Most importantly, they should not discontinue thyroid medication without first speaking with their healthcare provider.
