The FDA has approved Weltruza, a once-monthly olanzapine injection for adults with schizophrenia. Explore its benefits, clinical trial findings, safety information, and treatment considerations.

Introduction

The US Food and Drug Administration (FDA) has approved Weltruza (olanzapine) extended-release injectable suspension, a new once-monthly subcutaneous treatment for adults with schizophrenia. Announced by Teva Pharmaceuticals on October 9, 2026, the approval introduces a long-acting injectable option designed to address some of the challenges associated with daily oral medication.

Weltruza delivers olanzapine, an established antipsychotic medication, through an injection under the skin. According to Teva, the treatment is designed to provide sustained medication release without requiring loading doses, additional oral medication during initiation, or routine post-injection monitoring.

The approval represents a development in schizophrenia treatment, where maintaining consistent medication use can be challenging. However, as with any prescription medicine, the potential benefits must be considered alongside safety information and individual treatment needs.

What Is Weltruza?

Weltruza is a long-acting injectable formulation of olanzapine, an atypical antipsychotic used to treat schizophrenia. Unlike daily oral tablets, this formulation is administered once a month by subcutaneous injection.

Teva developed Weltruza to provide an alternative for adults who may find daily medication schedules difficult to maintain. Consistent treatment can be an important part of managing schizophrenia, although treatment outcomes vary between individuals.

According to the company announcement, Weltruza will be available in three strengths: 318 mg, 425 mg, and 531 mg. These doses correspond to the approved daily oral olanzapine dose ranges of 10 mg, 15 mg, and 20 mg, respectively.

The product uses SteadyTeq, a proprietary copolymer technology developed by Medincell. This technology is designed to provide controlled, sustained release of olanzapine following injection.

Teva stated that US availability was expected in the weeks following the October 2026 announcement.

Why Does This Approval Matter?

Schizophrenia is a complex mental health condition that can affect thinking, emotions, behaviour, and daily functioning. Symptoms may include hallucinations, delusions, disorganised speech, and difficulties with cognition.

Treatment often involves long-term medication and ongoing clinical support. However, taking medication every day can create practical challenges for some people, potentially contributing to treatment interruptions and an increased risk of symptom recurrence.

Long-acting injectable antipsychotics offer an alternative approach by reducing the frequency of medication administration. Instead of taking a tablet every day, patients receive medication at scheduled intervals under the supervision of a healthcare professional.

Weltruza introduces a monthly subcutaneous olanzapine option. Teva says the formulation does not require loading doses, oral supplementation, or post-injection monitoring as part of its administration approach.

These characteristics may be relevant to treatment planning for selected patients. Nevertheless, suitability depends on a person's medical history, previous response to treatment, preferences, and the advice of their treating clinician.

What Did the SOLARIS Clinical Trial Find?

The FDA approval was supported by results from the Phase 3 SOLARIS clinical trial, which evaluated the efficacy, safety, and tolerability of extended-release olanzapine administered under the skin.

During the first eight weeks, 675 participants aged 18 to 64 with schizophrenia were assigned to receive one of three Weltruza doses or a placebo.

The study assessed changes in symptoms using the Positive and Negative Syndrome Scale (PANSS). Researchers also evaluated illness severity through the Clinical Global Impression of Severity (CGI-S) scale and personal and social functioning through the Personal and Social Performance (PSP) scale.

According to Teva's announcement, the trial demonstrated overall improvement in schizophrenia symptoms, a significant reduction in illness severity, and improvements in personal and social functioning.

The company also reported that the treatment's systemic safety profile was consistent with that of oral olanzapine.

The study included a further 48-week treatment period to assess longer-term safety and tolerability. These findings form part of the evidence supporting the approval.

Although the reported results are encouraging, individual responses can differ. Patients should discuss the expected benefits and limitations with their healthcare professionals.

Potential Benefits of Monthly Treatment

Weltruza may offer several practical considerations for adults whose treatment plans include olanzapine.

Less frequent administration: Monthly injections can reduce the need to remember daily oral doses, although patients must still attend scheduled appointments.

An established medication: The active ingredient, olanzapine, has been used in schizophrenia treatment for decades.

No oral supplementation during initiation: According to Teva, the approved formulation does not require additional oral olanzapine during treatment initiation.

A different administration option: A subcutaneous injection may provide an alternative for patients and clinicians considering long-acting injectable treatment.

These features do not mean that Weltruza is suitable for everyone. Treatment decisions should reflect individual circumstances, clinical history, potential side effects, and patient preferences.

Safety Information and Possible Side Effects

Weltruza can cause serious side effects and requires careful medical assessment.

The most common adverse events reported in the SOLARIS trial included increased weight, sleepiness, headache, constipation, and injection-site reactions.

Weight gain and metabolic changes were also observed, consistent with the known safety profile of oral olanzapine. These changes can include elevated blood sugar, diabetes, and alterations in blood lipid levels.

Other serious risks described in the prescribing information include neuroleptic malignant syndrome, tardive dyskinesia, low blood pressure, falls, seizures, difficulty swallowing, and problems regulating body temperature.

The product information also warns about drug reaction with eosinophilia and systemic symptoms (DRESS), a potentially life-threatening reaction.

Post-injection delirium or sedation syndrome has occurred with another extended-release olanzapine formulation administered intramuscularly. It is not known whether this syndrome can occur with Weltruza, which is administered subcutaneously.

Weltruza is not approved for treating dementia-related psychosis in older adults. Antipsychotic medicines carry an increased risk of death in elderly patients with this condition.

Patients should tell their healthcare professionals about existing medical conditions, pregnancy or breastfeeding, and all medicines and supplements they use. Monitoring and follow-up should be determined by the treating clinician.

What Happens Next?

Weltruza's FDA approval adds another option to the range of treatments available for adults with schizophrenia in the United States.

Teva also reported that the European Medicines Agency had accepted a marketing authorisation application for its olanzapine long-acting injectable candidate in May 2026. At the time of the announcement, the product had not been approved by the EMA.

Further experience with the treatment will help clinicians understand how it fits into individual care plans and how its benefits and risks compare with other available options.

Conclusion

The FDA approval of Weltruza marks a development in long-acting injectable treatment for adults with schizophrenia. Its once-monthly administration, sustained-release formulation, and reported clinical trial outcomes may make it a relevant option for some patients.

However, the treatment also carries important safety risks, including weight gain, metabolic changes, and other potentially serious adverse effects. Decisions about starting, continuing, or changing antipsychotic medication should always be made with a qualified healthcare professional.

As Weltruza enters the US market, its role in schizophrenia care will depend on clinical judgement, patient needs, safety monitoring, and real-world treatment experience.
