Brazilian Conjoined Twins Die Minutes After Final Separation Operation
The story of Heloísa and Helena, twin sisters born joined at the head, has ended in tragedy after an extraordinarily complex attempt to separate them. The two-year-old girls died on August 15, 2026, during the fifth and final stage of a surgical process at Hospital das Clínicas in Ribeirão Preto, Brazil.
Their case had required more than a year of detailed planning, multiple operations and the work of a large multidisciplinary medical team. More than 50 healthcare and support professionals were involved in the final procedure, which lasted around 15 hours. Despite the team's efforts, the girls could not survive the final stage of separation.
A rare and exceptionally difficult condition
Heloísa and Helena were craniopagus twins, a term used for conjoined twins who are connected at the head. Their condition was particularly complicated because they shared brain tissue and blood vessels.
Separating twins in this situation is one of the most demanding procedures in paediatric surgery. Doctors must carefully identify which structures belong to each child and determine how shared blood vessels and other tissues can be divided without causing catastrophic damage.
For Heloísa and Helena, doctors chose to divide the process into several operations rather than attempt everything in a single procedure. The strategy was intended to gradually separate shared structures while giving the children's bodies time to recover between stages.
Five surgeries over more than a year
The girls began their surgical journey in August 2025. Their first operation achieved roughly a quarter of the planned separation, while later procedures progressively divided more of the shared structures.
By their third surgery, doctors reported that approximately 75 percent of the shared brain tissue and blood vessels had been separated. The fourth operation had a different purpose. Surgeons placed tissue expanders beneath the skin, gradually creating additional tissue that could later be used to close the girls' skulls after complete separation.
This staged approach illustrates just how much preparation was required. Imaging, surgical planning and advanced modelling were used to help doctors understand the anatomy of the twins and prepare for each stage.
The hospital had already treated two other pairs of craniopagus twins. Those earlier cases resulted in successful separations, giving the medical team valuable experience with an exceptionally rare surgical challenge.
The final operation
The fifth operation began on August 15. It was intended to complete the separation after the previous four procedures had prepared the girls' brains, blood vessels and skin.
According to reports from the medical team, the girls became severely unstable around the time the final structures connecting them were divided. One child experienced very low blood pressure while the other developed high blood pressure. The situation deteriorated rapidly after the separation.
Doctors described the problem as haemodynamic instability, meaning that the cardiovascular system was no longer able to maintain adequate blood flow to the body's vital organs. Both children subsequently suffered cardiac arrest.
Medical staff attempted resuscitation, but the efforts were unsuccessful. The sisters died only minutes apart.
A heartbreaking outcome for their family
The tragedy has been especially painful because the family and medical team had spent so long preparing for the final operation.
Their father, Amarildo Batista da Silva, expressed gratitude towards the doctors and staff who had cared for his daughters. He also offered a strikingly generous perspective on the outcome, saying that losing a child is a defeat but that saving a child is a victory, and that the medical team had achieved many more victories than defeats.
His comments reflect the extraordinary emotional difficulty surrounding complex paediatric surgery. Even when doctors have extensive experience, careful planning and advanced technology, some medical situations remain unpredictable.
The girls were buried in São José dos Campos, their family's home city, on August 16.
Why separation surgery is so challenging
The case also highlights the limits of modern medicine. Conjoined twins do not necessarily share the same anatomy, so every case presents different risks.
When twins are joined at the head, surgeons may have to deal with shared blood vessels, brain tissue, protective membranes and skull structures. Separating these structures can change blood flow to the brain and place enormous stress on both children's bodies.
The challenge does not necessarily end once the physical connection is divided. Doctors must also ensure that each child has enough blood supply and that the brain and other organs can continue functioning independently.
In Heloísa and Helena's case, the earlier surgeries had successfully achieved substantial progress. However, the final transition from a shared circulation and anatomy to two separate bodies proved too difficult for the girls to withstand.
A case that will remain important to medicine
Although the outcome was devastating, the treatment represents an extensive body of medical work and experience. The Hospital das Clínicas in Ribeirão Preto has now treated three cases involving twins joined at the head, according to Brazilian reports.
The earlier successful cases demonstrated that carefully staged procedures can make separation possible in some extremely complicated situations. The experience gained from each case can also contribute to future surgical planning and improve doctors' understanding of these rare conditions.
For Heloísa and Helena's family, however, the medical significance cannot overshadow their loss. They were two young children who had spent much of their lives undergoing treatment and recovering from major procedures.
Their final surgery was the culmination of months of preparation and hope. Although the operation did not produce the outcome everyone had wanted, the efforts of the medical team show the extraordinary lengths doctors can go to when treating some of the world's rarest and most complicated conditions.
