Atorvastatin and Simvastatin: What a Major New Study Says About Dementia Risk
A major new study has put statins back in the spotlight after researchers found that people with type 2 diabetes who took cholesterol-lowering statins appeared to have a lower risk of developing dementia.
The findings may be particularly relevant to people prescribed commonly used medicines such as atorvastatin and simvastatin. However, the results need to be interpreted carefully. The research found an association between statin treatment and a reduced risk of dementia, but it did not prove that taking statins directly prevents the condition.
The study was presented at the European Society of Cardiology Congress 2026 and published in The Lancet Regional Health – Europe. Researchers examined medical information from more than 132,000 people over a 15-year period, making it one of the largest studies of its type.
What did the researchers discover?
The research focused on people who developed type 2 diabetes and examined whether the timing of statin treatment was linked to their later risk of dementia.
Researchers compared people who did not begin statin treatment within five years of their diabetes diagnosis with those who started treatment relatively early. Early treatment was defined as beginning statins within one year, while later treatment referred to starting between one and five years after diagnosis.
The results suggested that people who started statins during the study period were around 10% less likely to develop dementia. Among those who began treatment within the first year following their type 2 diabetes diagnosis, the reduction was approximately 15%.
The association was observed in both men and women.
These figures have attracted attention because dementia prevention remains an important public health issue. Unlike some cardiovascular conditions, there is currently no treatment that can reliably reverse dementia once it has developed. This makes identifying potentially modifiable risk factors an important area of research.
Why does cholesterol matter?
Cholesterol is essential for the body, but having too much low-density lipoprotein cholesterol, commonly called LDL cholesterol, can contribute to the development of fatty deposits in arteries.
High LDL cholesterol is already an established risk factor for cardiovascular disease. Lowering LDL cholesterol is therefore an important part of preventing conditions such as heart attacks and strokes in people who are at increased risk.
The new research raises the possibility that managing cholesterol could also be relevant to brain health.
The researchers highlighted the importance of detecting and treating elevated LDL cholesterol from midlife. This may be especially significant for people with type 2 diabetes, because elevated LDL cholesterol is common in this group and diabetes itself is associated with an increased risk of cognitive decline and dementia.
That does not mean cholesterol is the only factor involved. Dementia is a complex group of conditions influenced by many different biological, genetic and lifestyle factors.
Why was early treatment important?
One of the most interesting aspects of the research was the apparent difference between people who started statins earlier and those who started later.
The study found that early statin use after a type 2 diabetes diagnosis was associated with a greater reduction in dementia risk than delayed treatment. According to the researchers, this could provide another reason for ensuring that eligible patients receive appropriate cholesterol treatment without unnecessary delays.
However, it is important not to interpret this as a recommendation for everyone with diabetes to start a statin immediately. Treatment decisions depend on an individual's overall cardiovascular risk, cholesterol levels, medical history, age and other factors.
A doctor or other qualified healthcare professional can assess whether a statin is appropriate and which treatment is best.
What are atorvastatin and simvastatin?
Atorvastatin and simvastatin are both statin medicines used to lower cholesterol.
Statins work by reducing the liver's production of cholesterol, which can lower levels of LDL cholesterol circulating in the bloodstream. They are widely used because reducing high cholesterol can help lower the risk of serious cardiovascular events in appropriate patients.
For people already taking one of these medicines, the new research is not a reason to change the dose or stop treatment without medical advice.
Instead, the findings add to a growing body of research examining how cardiovascular health and brain health may be connected.
Does this mean statins prevent dementia?
Not necessarily.
This is perhaps the most important point to take from the study.
Although the researchers found a relationship between statin use and a lower incidence of dementia, the study was observational. That means researchers analysed patterns in existing health records rather than randomly assigning people to receive statins or a placebo.
Observational research can identify important associations, but it cannot completely rule out other explanations.
For example, people who start statins may differ from people who do not start them in other ways. They may receive more regular medical care, have different health behaviours or have other characteristics that influence their risk of dementia.
The study authors themselves acknowledged this limitation. Professor Isabel Goncalves of the European Society of Cardiology said the findings suggest a modest reduction in dementia risk associated with starting statins soon after a type 2 diabetes diagnosis, but stressed that the observational design means the research cannot establish that statins prevent dementia.
Further research will therefore be needed to understand whether the relationship is causal.
What should people taking statins do now?
For people already prescribed atorvastatin, simvastatin or another statin, there is no suggestion from this study that they should stop taking their medication.
Statins are primarily prescribed to reduce cardiovascular risk, including the risk of heart attack and stroke. Any possible effect on dementia risk is an additional area being investigated by researchers.
Patients who have concerns about their medication should speak to their GP, pharmacist or specialist rather than stopping treatment on their own.
People with type 2 diabetes may also want to discuss their cholesterol levels and overall cardiovascular risk during routine appointments.
The broader message is that managing health risks earlier in life may have benefits beyond a single disease. Keeping LDL cholesterol under control, managing diabetes and addressing other established risk factors can all form part of a wider approach to protecting long-term health.
The bigger picture
The latest research offers an encouraging signal, particularly for people living with type 2 diabetes. A study involving more than 132,000 people found that statin use was associated with a lower risk of dementia, with the strongest association seen among those who started treatment relatively soon after their diabetes diagnosis.
But it would be premature to describe statins as a dementia-prevention treatment.
The findings are best viewed as another piece of evidence linking cardiovascular health, cholesterol management and cognitive health. They also highlight why researchers are increasingly interested in preventing dementia by addressing potentially modifiable risk factors earlier.
For now, the safest approach is straightforward: people should take prescribed medicines as directed, have their cholesterol and diabetes monitored appropriately, and discuss any concerns about treatment with a healthcare professional.
Future research may reveal whether the apparent reduction in dementia risk is directly caused by statins or whether other factors explain part of the association. Until then, the study provides an interesting and potentially important reason to continue investigating the relationship between cholesterol management and brain health.
